- (1) Community transition services are nonrecurring set-up expenses for a person who is transitioning from an institution to a Developmental Disabilities Program (DDP) waiver-funded home and community-based residential service.
- (2) The community transition service must be necessary to enable a person to establish a basic household.
(3) Community transition services are furnished only to the extent that:
- (a) they are reasonable and necessary;
- (b) clearly identified in the service plan;
- (c) the waiver recipient is unable to meet such an expense; and
- (d) the services cannot be obtained from other sources.
(4) Community transition services may include:
- (a) security deposits required to obtain a lease on an apartment or home;
- (b) essential household furnishings and moving expenses required to occupy and use a community domicile, including furniture, window coverings, food preparation items, and bath and bed linens;
- (c) set-up fees or deposits for utility or services access, including land line telephone, electricity, heating, and water;
- (d) services necessary for the individual's health and safety, such as pest eradication and one-time cleaning prior to occupancy;
- (e) moving expenses;
- (f) necessary home accessibility adaptations; or
- (g) activities to assess, arrange for, and procure needed resources as identified in the plan of care.
(5) Community transition services may not include:
- (a) monthly rental or mortgage expense;
- (b) food;
- (c) regular utility charges;
- (d) household appliances;
- (e) items that are intended for purely recreational purposes;
- (f) cellular telephone service; or
- (g) internet service.
- (6) Community transition service is not available to a person transitioning into residential settings that are owned or leased by a Developmental Disabilities Program (DDP) funded service provider.
- (7) The residential setting must be owned, leased, or rented by the person and must be considered the person's private residence.
- (8) Community transition services may not exceed $3,000 per person per transition.
