(1) Parenteral pharmaceuticals dispensed to patients must have a permanent label with the following information:
1. (a) the name and contact information of the pharmacy including a phone number which provides access to a pharmacist 24 hours per day, seven days per week;
2. (b) the date the product was prepared;
3. (c) the prescription identifying number;
4. (d) the patient's full name;
5. (e) the name of the prescribing practitioner;
6. (f) the directions for use including infusion rate and infusion device, if applicable;
7. (g) the name of each component, its strength, and amount;
8. (h) the expiration date of the product based on published data;
9. (i) the appropriate ancillary instructions such as storage instructions or cautionary statements including antineoplastic warning when applicable; and
10. (j) the identity of the pharmacist compounding and dispensing the product.
