1. (1) The following necessary forms for implementation of the act and these rules are available from the Department of Natural Resources and Conservation, P.O. Box 201601, Helena, Montana 59620-1601 and its Water Resources regional offices, or on the department's website. The department may revise as necessary the following forms to improve the administration of these rules and the applicable water laws:

   1. (a) Form No. 600P, "Permit Preapplication Meeting";
   2. (b) Form No. 600, "Application for Beneficial Water Use Permit," which must be submitted when an applicant desires to use surface water or groundwater that does not meet an exception to permit requirements under 85-2-306, MCA;
   3. (c) Form No. 602, "Notice of Completion of Groundwater Development";
   4. (d) Form No. 602E, "Request for Extension for Authorized Notice of Intent to Appropriate Groundwater," which must be submitted to request an extension under 85-2-306(3)(b), MCA;
   5. (e) Form No. 602I, "Notice of Intent to Appropriate Groundwater," which must be submitted to file a notice under 85-2-306(3)(b), MCA;
   6. (f) Form No. 603, "Well Log Report";
   7. (g) Form No. 605, "Application for Provisional Permit for Completed Stockwater Pit or Reservoir," which must be submitted for a pit or reservoir with a maximum capacity of less than 15 acre-feet and a total appropriation of less than 30 acre-feet per year;
   8. (h) Form No. 606P, "Change Preapplication Meeting";
   9. (i) Form No. 606, "Application to Change a Water Right";
   10. (j) Form No. 607, "Application for Extension of Time";
   11. (k) Form No. 608, "Water Right Ownership Update";
   12. (l) Form No. 611, "Objection to Application";
   13. (m) Form No. 613, "Fee Schedule for Water Use in Montana";
   14. (n) Form No. 617, "Project Completion Notice for Permitted Water Development";
   15. (o) Form No. 618, "Project Completion Notice for Change of a Water Right";

   17. (q) Form No. 626, "Application to Renew a Temporary Water Right Change";
   18. (r) Form No. 630, "Controlled Groundwater Area Petition";

   20. (t) Form No. 633, "Aquifer Test Data Form";
   21. (u) Form No. 634, "Notice of Change in Appropriation Right for a Replacement Well";
   22. (v) Form No. 635, "Notice of Change in Appropriation Right for Redundant Water Supply Well Construction," which must be submitted for redundant wells in a public water supply system as defined by 75-6-102, MCA;
   23. (w) Form No. 636, "Petition to Subordinate a State Water Reservation";
   24. (x) Form No. 637, "Reinstatement Request," which must be submitted to reinstate a permit or change authorization;
   25. (y) Form No. 638, "Water Reservation Application for Instream Flow," which must be submitted for instream flow water reservation applications allowed under the United States of America, Department of Agriculture, Forest Service-Montana Compact, Article VI, section B;
   26. (z) Form No. 640, "Certification of Water Right Ownership Update," which must be completed and submitted to the county clerk and recorder with a Realty Transfer Certificate when a water right is being divided or exempted (reserved) from the property;
   27. (aa) Form No. 641, "DNRC Ownership Update, Divided Interest," which must be submitted for a water right that will be divided;
   28. (bb) Form No. 642, "DNRC Ownership Update, Split and Sever of a Water Right," where a portion of the water right place of use is severed;
   29. (cc) Form No. 643, "DNRC Ownership Update, Severed Water Right," which must be submitted where a water right will be severed from the entire place of use;
   30. (dd) Form No. 644, "Notice of Change in Appropriation Right for a Replacement Point of Diversion," which must be submitted for replacement of surface water points of diversion under 85-2-429, MCA;
   31. (ee) Form No. 645, "Permit Registration for Groundwater Use within the National Park Service Compact Area," which must be submitted for groundwater developments with a maximum use of 35 GPM or less, not to exceed ten acre-feet per year;
   32. (ff) Form No. 646, "Geothermal Heating/Cooling Notice of Completion," which must be submitted for groundwater developments for a geothermal purpose with a maximum use of 350 GPM;
   33. (gg) Form No. 647, "Notice of Completion of Emergency Fire Protection Development," which must be submitted for groundwater developments by local governmental fire agencies organized under Title 7, chapter 33, MCA, for emergency fire protection;
   34. (hh) Form No. 650, “Temporary Lease of Appropriation Right,” which must be submitted to apply for a temporary lease under 85-2-428, MCA;
   35. (ii) Form No. 651, "Petition to Modify a Permit or Change Authorization";
   36. (jj) Form No. 652, "Petition for Stream Depletion Zone";
   37. (kk) Form No. 653, "Variance Request," which must be submitted for variance requests in ARM 36.12.123;
   38. (ll) Form No. 654, "Public Comment on a Draft Preliminary Determination," which must be submitted for public comment under 85-2-307(4), MCA;
   39. (mm) Form No. 655, "Application Amendment Form," which must be submitted to modify an application under ARM 36.12.1401;
   40. (nn) Form No. 656, "Notice of Change in Appropriation Right for Stock Tanks," which must be submitted for a notice filed under 85-2-429, MCA;
   41. (oo) Form No. 657, "Notice of Change in Appropriation Right for Municipal Use," which must be submitted for a notice filed under 85-2-429, MCA;
   42. (pp) Form No. 658, "Petition for Reduction, Modification, or Revocation of a Provisional Permit," which must be submitted for a petition filed under 85-2-313, MCA;
   43. (qq) Form No. 659, "Response to Petition for Reduction, Modification, or Revocation of a Provisional Permit," which must be submitted for a response filed under 85-2-313, MCA; and
   44. (rr) Form No. 670, "Consent to Approval of Application," which must be submitted for a consent to approval filed under 85-2-311 or 85-2-402, MCA.
