1. (1) Claims examiners must be certified by the department upon the following:

   1. (a) completion of the application form provided by the department;
   2. (b) payment of the required fees; and
   3. (c) satisfactory completion of an examination.
2. (2) Certification is for a two-year period. The certification date is the date of the successfully completed examination.
