- (1) A plan No. 2 or plan No. 3 employer shall submit the first report of injury/occupational disease form to its insurer within six days of notice of an accident, injury, or occupational disease. If a first report is improperly sent to the department, it will be forwarded to the insurer, if any.
- (2) An employer not covered by workers' compensation shall submit the report to the department.
- (3) An employee may submit a report to the department.
