1. (1) The self-insurer shall notify the department in writing:

   1. (a) 60 days prior to implementing changes that may affect its self-insurance status including but not limited to:

      1. (i) name, controlling ownership, legal status, change in proposed employer group membership, or permanent employment location;
      2. (ii) permanent increase or decrease of more than 25% in the number of employees in Montana; or
      3. (iii) changes in the policies, procedures or administration of its self insurance program; and

   2. (b) within 30 days subsequent to:

      1. (i) adverse material change in financial status;
      2. (ii) adverse material change in liabilities;
      3. (iii) changes in excess coverage;
      4. (iv) permanent reductions, shutdowns, suspensions, or closures of Montana operations; or
      5. (v) new or additional location of employment in Montana as a result of a significant change of operations.
