(1) Application for certification requires the following documentation:
- (a) a completed SIF application form signed by the applicant; and
- (b) medical evidence of permanent impairment signed by a physician qualified to complete impairment ratings.
- (2) When the department approves an application, the department shall notify the applicant and referring agent, if applicable, and provide the applicant with a letter of SIF certification.
- (3) The department shall retain an incomplete application for a period of one year. When an application remains incomplete one year after submission, the department shall deny the application.
- (4) When the department denies an application, the department shall issue an order informing the applicant and referring agent, if applicable.
