1. (1) The program requirements of the NAP are that the licensee shall:

   1. (a) agree to evaluations and testing necessary to determine treatment and monitoring needs while a part of the NAP;
   2. (b) sign a contract with the NAP and comply with said contract and all requirements as indicated in the NAP participant handbook; and
   3. (c) be responsible for all costs for treatment and monitoring.
