1. (1) All applicants for licensure with an out-of-state license shall submit a completed application on a form prescribed by the department.

2. (2) The application shall be deemed incomplete until all of the following are received by the board, in conjunction with the application:

   1. (a) the appropriate fee;
   2. (b) a copy of an official transcript or certificate of completion evidencing to the board's satisfaction that the applicant has a high school diploma or its equivalent;
   3. (c) proof that the applicant is eighteen years of age or older; and

   4. (d) verification of an active license, certification, or registration in good standing from another state or jurisdiction, whose current requirements are substantially equivalent to Montana requirements for licensure, including:

      1. (i) proof of completing a massage therapy program demanding a course of studies that includes, at a minimum, 500 hours; and
      2. (ii) a passing score on either the MBLEx, NCETMB, NESL, or the NCETM examination, or a state examination deemed to be equivalent.
