1. (1) An applicant shall have one year from the date of department approval to take the examination for which the application was approved. If the examination is not taken within that one-year period, the applicant will be required to submit a new application, provide written verification of experience, and pay the applicable fees.

2. (2) All applications shall be approved or disapproved on a case-by-case basis as the department may deem proper, according to the following criteria:

   1. (a) complete application submitted;
   2. (b) fees paid;
   3. (c) acceptable work experience verified; and
   4. (d) past disciplinary actions reviewed and considered, if applicable.
