1. (1) A licensee shall notify the director of any of the following changes in the licensed business:

   1. (a) any change of business structure;
   2. (b) material change of ownership;
   3. (c) voluntary or involuntary business closure;
   4. (d) changes in the officers;
   5. (e) changes in the board of directors;
   6. (f) change of business address; and
   7. (g) any change of bank account information.
2. (2) Pursuant to the licensee's contract, a licensee shall notify the director of any change, move, or removal of lottery terminal, equipment, and/or peripherals.
