PURPOSE: This rule describes the forms which must be filed by a health maintenance organization with the Department of Insurance for approval prior to use. This rule is promulgated pursuant to sections 354.405 and 354.485, RSMo.

1. (1) The following forms shall not be delivered or issued for delivery in this state until they have been submitted to the Missouri Department of Insurance and approved by the director:

   1. (A) Group and individual contracts;
   2. (B) Evidence of coverage to be issued to the enrollees;
   3. (C) Application forms;
   4. (D) Enrollment forms;
   5. (E) Riders;
   6. (F) Amendments;
   7. (G) Endorsements; and
   8. (H) Any other forms which are intended to become part of a contract which is provided to an enrollee or group subscriber.
2. (2) Each filing shall be made in accordance with the procedures outlined in 20 CSR 400- 8.200.
