PURPOSE: This rule outlines the process for renewing a practitioner’s license and/or an establishment license.
$200 form containing updated information since the preceding $300 application/renewal period, as well as the required fee for $200 renewal. $300
$100 division does not constitute acceptance of the renewal $100 application. Any practitioner or holder of an establishment license who fails to renew the license by the expiration date shall not perform any act for which a license is required. $100
$100 said license by the expiration date will cause the license to $120 expire. Within two (2) years of the expiration date, the owner/ $120 operator may submit payment of the renewal fee, provide a completed renewal form as provided by the division, and a statement regarding whether the establishment has been $ 5 $ 25 operating since the date of expiration.
