PURPOSE: This rule establishes the fees for both clinical and baccalaureate social workers. (1) The following fees are established by the committee and are payable in the form of a cashier’s check, personal check, or money order:
- (A) Application/Initial License Fee
- (B) Registration of Supervision Fee
- (C) Two- (2-) Year License Renewal Fee
- (D) Delinquent Fee for Failure to Obtain a License or Timely Renew a License
- (E) Restoration of Lapsed License Fee
- (F) Inactive Status
- (G) Reciprocity Application Fee
- (H) Wall-Hanging Replacement Fee
- (I) Insufficient Funds Check Charge Fee
- (J) Duplicate License Fee
- (K) Verification of Licensure
