PURPOSE: This rule establishes the obligation of licensees to inform the State Committee of Psychologists of their changes of address.
- (1) Within thirty (30) days of the effective date of the change, a licensee must inform the State Committee of Psychologists of all changes in the mailing address as it appears on the licensee’s license by contacting the office of the State Committee of Psychologists.
- (2) A licensed psychologist whose name is legally changed shall notify the committee within thirty (30) days of the name change and provide a copy of an appropriate document substantiating the name change.
