PURPOSE: This rule establishes and fixes the various fees and charges authorized by Chapter 338, RSMo.

1. (1) The following fees are established by the State Board of Pharmacy:

   1. (A) Licensure by Examination Fee

      1. 1. Exam candidate shall contact the

National Association of Boards of Pharmacy and pay any fee required directly by that agency.

1. (B) Licensure By Transfer of License (Reciprocity)
2. (C) Original Pharmacy Permit Fee
3. (D) Pharmacist License Renewal Fee
4. (E) Pharmacy Permit Renewal Fee
5. (F) Delinquent Pharmacist Renewal Fee (in addition to the Pharmacist License Renewal Fee)
6. (G) Duplicate License/Permit/ Registration Fee
7. (H) Change of Pharmacy or Drug Distributor Name Fee
8. (I) Fee for Retake of Multistate Pharmacy Jurisprudence Examination (MPJE)
9. (J) Foreign Graduate Preliminary Filing Fee (Candidates for licensure by examination, who are graduates of schools/ colleges of pharmacy not accredited by the board)
10. (K) Change of Pharmacy or Drug Distributor Location Fee
11. (L) Original Pharmacy Distributor/ Wholesale Drug Distributor License Fee (includes both temporary and permanent license)
12. (M) Pharmacy Distributor/ Wholesale Drug Distributor License Renewal Fee

13. (N) Original Drug Distributor (Manufacturer) Registration Filing Fee

    1. (O) Renewal of Drug Distributor
    2. (P) Original Intern Pharmacist
    3. (Q) Intern Pharmacist License
    4. (R) Temporary Pharmacist License
    5. (S) Fingerprint Fee for Criminal
    6. (T) Pharmacy Technician
    7. (U) Pharmacy Technician Annual

1. (3) The provisions of this rule are declared

$ 20 severable. If any fee fixed by this rule is held invalid by a court of competent jurisdiction or $ 25 by the Administrative Hearing Commission, the remaining provisions of this rule shall remain in full force and effect, unless other- $ 150 wise determined by a court of competent jurisdiction or by the Administrative Hearing Commission.
