PURPOSE: This rule establishes and fixes the various fees and charges authorized by Chapter 336, RSMo.

1. (1) The following fees are established by the State Board of Optometry:

   1. (A) Application Fee
   2. (B) Missouri Law Exam Fee
   3. (C) Biennial Renewal Fee
   4. (D) Late Fee
   5. (E) Reactivation Fee
   6. (F) Duplicate Certificate Fee
   7. (G) Certification of Corporation Fee
   8. (H) Reciprocity Certification Fee
   9. (I) Computer Print-Out of Licensees Fee
   10. (J) Pharmaceutical Certification Fee (for certification to use DPA and therapeutic pharmaceutical agents)
   11. (K) Uncollectible Fee (uncollectible check or other uncollectible financial instrument)

   12. (L) Law Book Requests Fee

       1. (M) Biennial Continuing Education Sponsor Fee $ 25
       2. (N) Continuing Education Penalty Fee (reporting continuing education hours obtained after the end of the reporting period) $ 50

       *This fee also includes the license fee and the pharmaceutical certification fee. **When administered separately from the National Board of Examiners in Optometry (NBEO) Patient Assessment and Management (PAM) exam. ***This fee will not apply to the initial copy of the law book which is automatically mailed to all applicants for licensure or to accredited schools of optometry. Furthermore, this fee will not be charged to licensees or any other individual for additions or corrections to the law book after the initial copy is mailed.

       1. (2) All fees are nonrefundable.
       2. (3) The provisions of this rule are declared severable. If any fee fixed by this rule is held invalid by a court of competent jurisdiction or by the Administrative Hearing Commission, the remaining provisions of this rule shall remain in full force and effect, unless otherwise determined by a court of competent jurisdiction or by the Administrative Hearing Commission.
