PURPOSE: This rule establishes the fees for the Division of Professional Registration and the Missouri Board of Occupational Therapy.

1. (1) The Division of Professional Registration establishes the following fees which are nonrefundable:

   1. (A) Application for Licensure as an Occupational Therapist Fee
   2. (B) Application for Licensure as an Occupational Therapy Assistant Fee
   3. (C) Application for Limited Permit Fee
   4. (D) Biennial Occupational Therapist License Renewal Fee
   5. (E) Biennial Occupational Therapy Assistant License Renewal Fee
   6. (F) Late Renewal Penalty Fee
   7. (G) Biennial Inactive Occupational Therapist License Renewal Fee
   8. (H) Biennial Inactive Occupational Therapy Assistant License Renewal Fee
   9. (I) Endorsement Verification to Another Jurisdiction Fee
   10. (J) Duplicate License/Limited Permit Fee
   11. (K) Replacement Wall-Hanging Fee
   12. (L) Return Check Fee Charge

(1) A licensed occupational therapist, occupational therapy assistant or a limited permit holder shall ensure the board has the current legal name and address of the licensee or limited permit holder. (2) A licensed occupational therapist, occupational therapy assistant or limited permit holder whose name is changed by marriage or court order shall notify the board within thirty (30) days of the name change and provide a copy of the appropriate document verifying the name change.
