PURPOSE: This rule defines the procedure for change of sponsorship.

1. (1) An institution assuming the sponsorship of an approved program of practical nursing shall notify the board in writing within ten
2. (10) working days after the change of sponsorship.
3. (2) A Change in Sponsorship Form provided by the board shall be completed and returned within thirty (30) days of receipt of form.
4. (3) Any proposed changes that affect the criteria included in 4 CSR 200-3.010(1)(C)1. A.–C. must be approved by the board prior to implementation.
5. (4) Program documents shall be changed to indicate the appropriate sponsor. The board may issue a Certificate of Approval indicating the change in sponsorship, if appropriate.
