PURPOSE: This rule defines student records required to be kept by programs of professional nursing.

1. (1) Transcripts.

   1. (A) Transcripts of all courses attempted or completed by each student attending the program shall be maintained permanently. Courses shall be listed in chronological order.

   2. (B) The final transcript shall identify the following:

      1. 1. Date of admission, date of separation

from the program and hours/credits/units earned and the diploma/degree awarded; and

1. 2. Transferred credits, including course

titles, credits earned, and the name and location of the credit-granting institution.

1. (C) Seal of the school and signature of the current administrator or registrar shall be affixed to all official transcripts.

1. (2) School Records.

   1. (A) Student records shall be stored in an area which is documented as theft resistant and where confidentiality can be ensured.
   2. (B) Transcripts, including microfiche and computer files, shall be stored in documented fire-resistant files/areas.
   3. (C) The nursing program shall maintain records as required for approval.
