PURPOSE: This rule establishes and fixes the various fees and charges for the Board of Therapeutic Massage.

1. (1) All fees shall be paid by cashier’s check, personal check, business check, money order, or other method approved by the division and must be made payable to the Board of Therapeutic Massage.
2. (2) No fee will be refunded should any license be surrendered, suspended or revoked during the term for which the license is issued.

3. (3) The fees are established as follows:

   2. (B) Business License Renewal Fee

      1. 1. Late Renewal Fee
   3. (C) Certified Mentor Fee
   4. (D) Insufficient Funds Check Charge Fee
   5. (E) Massage Therapist Application Fee

   6. (F) Massage Therapist Renewal Fee

      1. 1. Late Renewal Fee 1–30 days
      2. 2. License Reinstatement Fee 31 days—2 years
      3. 3. Late Continuing Education Fee
   7. (G) Provisional License Application Fee
   8. (H) Provisional License Renewal Fee
   9. (I) Criminal Background Check Fee Amount to be determined by the Missouri State Highway Patrol
   10. (J) Student License Fee
   11. (K) Education Review Fee
4. (4) All fees are nonrefundable.
