PURPOSE: This rule establishes the fees for speech pathologists or audiologists, or both.

1. (1) The following fees are established by the Advisory Commission for Speech-Language Pathologists and Audiologists and are payable in the form of a cashier’s check or money order:

   1. (A) Licensure Application Processing Fee $50.00
   2. (B) Biennial Licensure Renewal Fee —Odd Numbered Years (personal checks acceptable) $50.00
   3. (C) Reinstatement Fee $25.00
   4. (D) Continuing Education Extension Fee (personal checks acceptable) $15.00
   5. (E) Returned Check Fee $25.00

2. (2) The following fees apply to speech-language pathology and audiology aides:

   1. (A) Registration Application Processing Fee $25.00
   2. (B) Biennial Registration Renewal Fee —Odd Numbered Years (personal checks acceptable) $20.00
   3. (C) Reinstatement Fee $10.00
   4. (D) Returned Check Fee $25.00

3. (3) The following fees apply to speech-language pathology and audiology provisional licensees:

   1. (A) Provisional Licensure Application Processing Fee $50.00
   2. (B) Provisional Licensure Renewal Fee $50.00
   3. (C) Returned Check Fee $25.00

4. (4) The following fees apply to speech-language pathology assistants:

   1. (A) Registration Application Processing Fee $25.00
   2. (B) Biennial Registration Renewal Fee— Odd Numbered Years (personal checks acceptable) $20.00
   3. (C) Reinstatement Fee $10.00
   4. (D) Returned Check Fee $25.00
5. (5) All fees are nonrefundable.
6. (6) The provisions of this rule are declared severable. If any fee fixed by this rule is held invalid by a court of competent jurisdiction or by the Administrative Hearing Commission, the remaining provisions of this rule shall remain in full force and effect, unless otherwise determined by a court of competent jurisdiction or by the Administrative Hearing Commission.
