PURPOSE: This rule outlines the requirements and procedures for notifying the board of name and address changes.

1. (1) An individual practicing under a license issued by the board shall ensure that the current license bears the current legal name of that individual.

2. (2) A registered geologist or geologist-registrant in-training whose name is changed by marriage or court order shall promptly—

   1. (A) Notify the board of such change and provide a copy of the appropriate document indicating such change within thirty

3. (30) days of the name change; and

   1. (B) Request a new license bearing the individual’s new legal name.
4. (3) A registered geologist or geologist-registrant in-training whose address has changed must inform the board of all such changes by notifying the board’s office in writing within sixty
5. (60) days of the effective date of the change.
