PURPOSE: This rule outlines the requirements and procedures for the renewal of embalmers, funeral directors, funeral directors limited, and funeral establishments.

1. (1) The biennial license renewal date for licensed embalmers, licensed funeral directors, and licensed funeral director limited is June 1. The biennial license renewal date for licensed funeral establishments is January 1.
2. (2) Embalmers, funeral directors, funeral director limited, and funeral establishment licenses which are not renewed will be notified that their licenses have expired. The holder of an expired license may reinstate the license within two (2) years of the renewal date after the proper forms have been completed and applicable fees have been paid. Any embalmer funeral director, and funeral establishment license not renewed within two (2) years shall be void.
3. (3) The licensee’s failure to receive the renewal notice shall not relieve the licensee of the duty to pay the renewal fee and renew his/her license.
