PURPOSE: This rule describes the board’s operation and procedures for a name change.

1. (1) Whenever used in Title 4, Division 70 of the Code of State Regulations, the word “board” means the State Board of Chiropractic Examiners.
2. (2) Unless otherwise provided by statute or regulation, regular and special meetings of the board shall be governed by Roberts’ Rules of Order.
3. (3) When a licensee or an applicant for licensure changes his/her name in any way, the licensee or applicant shall send to the board a copy of the official document granting the name change.
