PURPOSE: This rule establishes fees charged for laboratory services performed by Animal Health Laboratories. (1) The Animal Health Diagnostic Laboratories are operated by and under the direction of the state veterinarian. The mailing addresses of the Animal Health Laboratories are as follows:

1. (A) Cooperative State & Federal Veterinary Diagnostic Laboratory, 216 El Mercado Plaza, Jefferson City, MO 65109 (this laboratory performs primarily cooperative state and federal program tests);
2. (B) Veterinary Diagnostic Laboratory, 701 North Miller Avenue, P.O. Box 2510, Springfield, MO 65802-2510; and
3. (C) Northwest Missouri Veterinary Diagnostic Laboratory, 307 West Grand, P.O. Box 377, Cameron, MO 64429.

4. (2) No fees will be charged for tests for diseases which are included in a state and federal cooperative program. Fees for nonprogram services performed at the Animal Health Laboratories are as follows:

1. (6) Uncollectible checks or other uncollectible financial instruments will be charged an additional twenty-five dollars ($25).
2. (7) All samples must be submitted on official forms and must be complete and legible. Test charts and samples improperly submitted may be returned to the veterinarian for rectification.
