Mo. Code Regs. Ann. tit. 19, § 73-2.020
PURPOSE: This rule specifies the minimum requirements for licensure as a nursing home administrator in Missouri.

(1) An applicant interested in becoming a licensed nursing home administrator shall obtain an application form from the board. The application form, MO 580-2518 (03-11), Application for Licensure NHA, is incorporated by reference in this rule and is available on the web at www.health.mo.gov/information/ $150 boards/bnha or by contacting the board at PO Box 570, Jefferson City, MO 65102, (573) 751-3511. This rule does not incorporate any subsequent amendments or additions. The application $ 50 $100 shall be completed and returned to the board with the fee $ 50 referenced in 19 CSR 73-2.015. Information provided in the application shall be attested by signature to be true and correct to the best of the applicant’s knowledge and belief. $ 25
