PURPOSE: This rule establishes and fixes the various fees and charges authorized by Chapter 344, RSMo.

1. (1) The following fees are established by the Board of Nursing Home Administrators:

   1. (A) Application Review Fee (including reciprocity) $100.00;
   2. (B) National and State Exam Fee or National Exam Fee (when taken on one of the designated quarterly testing dates) $150.00;
   3. (C) State Exam Fee for Reciprocity Candidates and for Candidates Needing to Retake the State Exam $ 50.00;
   4. (D) License Renewal Fee $ 50.00;
   5. (E) License Renewal Late Penalty Fee (This fee is in addition to the renewal fee listed in subsection (1)(C)) $ 25.00;
   6. (F) Duplicate License Fee $ 5.00;
   7. (G) Single Offering Fee (per requested clock hour) $ 10.00; and
   8. (H) Insufficient Funds Charge $ 25.00.
2. (2) All fees must be made payable to the director of revenue in the form of a cashier’s check, company check or money order.
3. (3) All fees are nonrefundable.
4. (4) The provisions of this rule are declared severable. If any fee fixed by this rule is held invalid by a court of competent jurisdiction or by the Administrative Hearing Commission, the remaining provisions of this rule shall remain in full force and effect, unless otherwise determined by a court of competent jurisdiction or by the Administrative Hearing Commission.
