Mo. Code Regs. Ann. tit. 13, § 110-3.010
PURPOSE: The purpose of this rule is to standardize the investigation into the pertinent circumstances of the youth’s background to facilitate appropriate treatment and aftercare planning.
(1) An individual treatment plan (ITP) shall be developed by the Division of Youth Services service coordinator for the purpose of meeting individual youth and family needs. The ITP shall also serve to record case activity and fulfill requirements for official notifications.
(A) ITP procedures are as follows:
ate a written ITP within thirty (30) days of the commitment date. The treatment plan should involve the youth and his/her parent or guardian. The ITP shall be submitted within forty-five (45) days of commitment, and distributed to the youth, family, court and facility;
is encouraged;
with youth in residential care at least once per month and shall meet with the youth on aftercare twice per month;
shall include, but not be limited to:
health, criminal, or division of family services case information);
the preliminary possibilities for the youth’s placement and needs of the youth to be considered for aftercare placement.
