PURPOSE: This rule sets forth the requirements for a Class D license application.

1. (1) An application for a Class D license must include, on a form prepared by the commission:

   1. (A) The name, address and telephone number of the applicant;
   2. (B) The name, position, address, telephone number and authorized signature of an individual to whom the commission may make inquiry; and

   3. (C) The information required by chapter 13.

2. (2) The application for Class D license must be accompanied by an applicant’s affidavit conforming to the requirements of 12 CSR 50-13.030.
