PURPOSE: The purpose of this rule is to require disclosure of any assistance in the preparation of the Class A license application.

1. (1) An applicant for a Class A license must disclose the names, addresses and telephone numbers of all individuals who assisted the applicant in the preparation of its application under these rules.
