PURPOSE: The purpose of this rule is to set forth the requirements for a Class A license application.

1. (1) An application for a Class A license must include, on a form prepared by the commission:

   1. (A) The name, address and telephone number of the applicant;
   2. (B) The name, position, address, telephone number and authorized signature of an individual to whom the commission may make inquiry; and
   3. (C) The information required by this chapter.
2. (2) The application for Class A license must be accompanied by an applicant’s affidavit conforming to the requirements of 12 CSR50- 10.030.
3. (3) The applicant shall be responsible to keep its pending application current at all times. The applicant shall notify the commission in writing within ten (10) days of any changes to any response in the application.
