PURPOSE: This rule requires licensees to notify the director of certain changes in their businesses.

1. (1) The licensee must notify the director of any of the following changes in his/her business:

   1. (A) Any change of business structure, that is sole proprietorship to partnership or corporation;
   2. (B) Changes of ownership for corporations, only sale of ten percent (10%) or more of the equity of the business;
   3. (C) Changes in the officers; and
   4. (D) Changes in the board of directors.
2. (2) The director shall require additional information when it is necessary to conduct background information on any persons added to the business.
3. (3) The director may require a payment of up to twenty-five dollars ($25) per person for any required investigation under this rule.
