PURPOSE: This rule sets forth the attached form as the form to be utilized in filing accident reports by persons required to make reports pursuant to section 303.040, RSMo.
- (1) This rule prescribes the specific form and instructions to be used in filing an accident report, MO 860-0427. This form, MO 860- 0427, is available on the Internet at http://www.dor.mo.gov/mvdl/drivers/forms/ 1140f.pdf. or by mailing a written request for the form to the Driver and Vehicle Services Bureau, PO Box 200, Jefferson City, MO 65105-0200. No form other than the one prescribed in this rule will be permitted in filing an accident report pursuant to the requirements in section 303.040, RSMo.
