PURPOSE: This rule establishes the criteria for placement of a permanent disability indicator on a driver or nondriver license.

1. (1) As used in this rule, the term “permanently disabled” means having a physical or mental impairment, which substantially limits one’s ability to perform one (1) or more major life activities and is permanent in nature, as determined by a licensed physician, physical therapist, or occupational therapist licensed pursuant to Chapter 334, RSMo, or other authorized licensed healthcare practitioner.
2. (2) As used in this rule, the term “healthcare practitioner” means a licensed physician, physical therapist, or occupational therapist licensed under Chapter 334, RSMo, or other authorized healthcare provider, licensed under the laws of the state of Missouri and 12 CSR 10-24

approved by the director of revenue.

1. (3) To obtain a permanent disability indicator on a driver or nondriver license, an applicant at the time of application for an initial, renewal, or duplicate driver or nondriver license shall present a medical statement, as provided in section (1), completed and certified by a healthcare practitioner as provided in section (2).
2. (4) The issuance of a permanent disability indicator or notation on a driver or nondriver license is not for the purpose of any determination of eligibility or entitlement to any benefit or accommodation.
