Subd. 2. Pharmacies.

1. (a) Each pharmacy, as defined in section 151.01, subdivision 2, shall provide the following information to a patient upon request:

   1. (1) the pharmacy's own usual and customary price for a prescription drug;
   2. (2) a record, including all transactions on record with the pharmacy both past and present, of all co-payments and other cost-sharing paid to the pharmacy by the patient for up to two years; and
   3. (3) the total amount of all co-payments and other cost-sharing paid to the pharmacy by the patient over the previous two years.
2. (b) The information required under paragraph (a) must be readily available at no cost to the patient.
