A person who holds a registration as a physician assistant shall, upon notification from the board, renew the registration by:

1. (1) submitting the appropriate fee as determined by the board;
2. (2) completing the appropriate forms;
3. (3) meeting any other requirements of the board;
4. (4) submitting a revised and updated practice setting description showing evidence of annual review of the physician-physician assistant supervisory agreement.
