The question of the adoption of a city manager plan of government may be submitted to the qualified electors of a city in the manner provided by K.S.A. 12-184. The ballots used for the submission of such question shall be as follows:

"Shall the governing body of the city (naming the city) appoint a city manager and fix by ordinance the salary of such city manager?"
