Sec. 2. The purpose of this rule is to prescribe the following for insurers and health maintenance organizations:

1. (1) The form for filing information with the commissioner, as required by IC 27-8-28-19 and IC 27-13-8-2(a).
2. (2) Requirements for notifying enrollees of grievance procedures.
3. (3) Requirements for filing, investigating, and resolving grievances and appeals.
