Sec. 4. Each sports wagering kiosk or sports wagering kiosk computer system shall be capable of generating a transaction report, which documents each attempted and completed transaction. Unless otherwise approved by the commission, the report shall include, at a minimum:

1. (1) the date and time;
2. (2) a description of the transaction;
3. (3) the value of credit card and debit card transactions;
4. (4) the value of currency dispensed and inserted;
5. (5) the value of all sports wagering vouchers dispensed and inserted; and
6. (6) the value of all sports wagering tickets dispensed and inserted.
