Sec. 9.2. (a) This rule applies to all riverboat licensees.

(b) Riverboat licensees must advise the enforcement agent, on a form prescribed or approved by the commission, when one (1) of the following events occurs with an occupational licensee:

1. (1) The occupational licensee's employment with the riverboat licensee is terminated for any reason.

2. (2) The occupational licensee:

   1. (A) is suspended by the riverboat licensee;
   2. (B) is on a leave of absence; or
   3. (C) transfers to another position with the riverboat licensee.

(c) The form utilized by the riverboat licensee to report the information set forth in subsection (b) must provide, at a minimum, the following information:

1. (1) The:

   1. (A) name;
   2. (B) address;
   3. (C) telephone number; and
   4. (D) occupational license number;
2. of the occupational licensee.
3. (2) The marital status of the occupational licensee.
4. (3) The date the change or action will take effect.
5. (4) The job title and position of the occupational licensee affected.
6. (5) The reason for the change or action taken.

7. (6) If the occupational licensee was terminated:

   1. (A) whether the termination was voluntary or involuntary; and
   2. (B) whether or not the riverboat licensee collected the identification badge from the occupational licensee.

8. (7) The:

   1. (A) signature of the individual completing the form for the riverboat licensee; and
   2. (B) date on which the form was completed.
9. The form must be submitted to the enforcement agent within fifteen (15) days of the occurrence of the change or action.

(d) Riverboat licensees must collect the identification badge issued by the commission to an occupational licensee when the occupational licensee's employment with the riverboat licensee is terminated for any reason. All identification badges collected by the riverboat licensee must be turned over to an enforcement agent within seven (7) days.
