Sec. 30. (a) Upon written request, from any person, to the board for a showing of compliance by any principal contractor, intermediate contractor, or subcontractor with IC 22-3-5, IC 22-3-5-1, and IC 22-3-5-2 as required by IC 22-3-2-14 and with IC 22-3-7-34(a) and IC 22-3-7-34(b) as required by IC 22-3-7-34(i), the board will issue a certificate showing such compliance as is reflected by its records.

(b) Each request must be accompanied by a preaddressed, stamped envelope for each party who is to receive a copy of the certificate.
