Sec. 65. (a) At the time of discharge, the child caring institution shall document the following in the child's case record:

1. (1) A written summary report of the care received by the child including any recommendations for the child and family.
2. (2) The date and reasons for discharge.
3. (3) The name, address, telephone number, and relationship of the person or agency to whom the child is released.

(b) The summary report shall be provided to the placing agency at the time of discharge.

(c) At the time of discharge, the child caring institution shall make a summary of health recommendations for the child available to the parents, guardian, placing agency, other individual, or agency to whom the child is released.
