Sec. 9. An optional resources attachment regarding resources may be attached to the ISP. If an optional resources attachment is used it may indicate the following:

1. (1) The funding supports the individual currently receives.
2. (2) The funding supports the support team discussed during the development of the ISP.
3. (3) The funding supports the individual does not desire to receive.
4. (4) The funding supports for which the individual has applied.
5. (5) Any funding supports for which the individual is on a waiting list.
