Sec. 6. (a) If an individual changes providers for any supported living service or support, the new provider shall:

1. (1) discuss with the individual the new provider's need to obtain a copy of the previous provider's records and files concerning the individual;
2. (2) provide the individual with a written form used to authorize the previous provider's release of a copy of the records and files concerning the individual to the new provider; and
3. (3) request the individual to sign the release form.

(b) Upon receipt of a written release signed by the individual, a provider shall forward a copy of all of the individual's records and files to the new provider no later than seven (7) days after receipt of the written release signed by the individual.
