Sec. 6. To be approved to provide community-based sheltered employment services, an applicant shall meet the following requirements:

1. (1) Be an entity.

2. (2) Be accredited by one (1) of following organizations:

   1. (A) The Commission on Accreditation of Rehabilitation Facilities (CARF) or its successor.
   2. (B) The Council on Quality and Leadership in Supports for People with Disabilities or its successor.
   3. (C) The Joint Commission on Accreditation of Healthcare Organizations (JACHO [sic., JCAHO]) or its successor.
   4. (D) The National Commission on Quality Assurance or its successor.
   5. (E) An independent national accreditation organization approved by the secretary.
3. (3) Be a not-for-profit entity.
4. (4) Certify that, if approved, the entity will provide community-based sheltered employment services using only persons who meet the qualifications set out in 460 IAC 6-14-5.
5. (5) Not be a community mental health center.
