Sec. 2. (a) This section applies to all providers.

(b) A provider shall maintain in the provider's office documentation of all services provided to an individual.

(c) Documentation related to an individual required by this article shall be maintained by the provider for at least seven (7) consecutive years.

(d) A provider shall analyze and update the documentation required by:

1. (1) the standards under this article applicable to the services the provider is providing to an individual;
2. (2) the professional standards applicable to the provider's profession; and
3. (3) the individual's ISP.

(e) A provider shall analyze and update the documentation at least every ninety (90) days if:

1. (1) the standards under this article do not provide a standard for analyzing and updating documentation;
2. (2) the professional standards applicable to the provider's profession do not provide a standard; or
3. (3) a standard is not set out in the individual's ISP.
