Sec. 5. The division shall maintain a file for each personal services attendant that contains the following:

1. (1) Comments related to the provision of attendant care services, including periodic reports on the quality of services provided by the personal services attendant, submitted by an individual in need of self-directed in-home care who has employed the personal services attendant; and
2. (2) The items described in section 4(a)(1) through 4(a)(4) of this rule.
