Sec. 2. (a) This rule establishes general application requirements for a permit to:
a well for oil and gas purposes.
(b) An application for a permit to:
a well for oil and gas purposes shall be made on a form prescribed by the division of reclamation.
(c) A permit application must be signed by the person designated on the application as a person who controls the well for oil and gas purposes. Upon a request by the division of reclamation, an authorized representative who signs on behalf of an owner, operator, or entity must furnish satisfactory evidence of authority.
(d) An applicant shall remit with the application a permit fee of two hundred fifty dollars ($250) in cash, by check, or by draft, payable to the department. A person may apply for an expedited review of the application for a permit, except for a Class II or noncommercial well, by submitting a permit fee of seven hundred fifty dollars ($750).
(e) An application must be accompanied by a survey certified by a professional surveyor registered under IC 25-21.5 showing the location of the proposed well for oil and gas purposes, giving the:
(f) For horizontal and directional wells, in addition to the requirements of subsection (e), the survey certified by a professional surveyor registered under IC 25-21.5 shall include the following:
(g) A bond, if required under 312 IAC 29-12-3, must accompany a permit application.
