Sec. 5. (a) The owner or operator shall monitor and record:

1. (1) maximum injection pressure;
2. (2) average injection pressure; and
3. (3) the injection volume;

for a Class II well on a weekly basis.

(b) The owner or operator shall summarize the data collected under subsection (a) on a monthly basis.

(c) The owner or operator shall file with the division of reclamation a quarterly report containing the information specified in subsections (a) and (b) on a form prescribed by the division of reclamation within thirty (30) days after the end of the quarter being reported.

(d) The owner or operator shall use the same form to indicate casing annulus pressure monitoring used instead of any other casing annulus pressure test performed.
